Read the release of liability

I waive and release Mo’s Soccer Academy (formerly Mo’s Soccer Camps), its directors, coaches, staff, independent contractors, and City of Dublin from all liability, claims, demands, and causes of action arising out of or related to any loss, personal injury (including death), disease, illness, or property loss that may be sustained or occur from participation in Mo’s Soccer Academy. I completely understand all the risks associated with participation in this program. I certify that the undersigned child is physically capable of participating in the Mo’s Soccer Academy and all related activities without any restrictions. I understand that any personal items lost or stolen will not be replaced by the Mo’s Soccer Academy. If my child is injured or becomes ill at camp and I cannot be reached, I authorize Mo’s Soccer Academy staff to seek and consent to emergency medical treatment on my behalf, and I am responsible for the cost of that treatment. I have listed any allergies, conditions and medications my child has, along with an emergency contact, on the registration form. CANCELLATION AND REFUNDS. If I cancel my child’s registration, the price paid for the sessions of that registration still to come (not including the card processing fee) is added to my family account as credit. That credit never expires and can be used by any of my children for any future session. Refunds are given only when Mo’s Soccer Academy cancels a session, and cover the part of it still to come. If a session is canceled because of the weather, whether it is refunded is at the discretion of Mo’s Soccer Academy. There is no refund or credit if my child leaves camp early because of illness, injury, breaking camp rules, homesickness, or dissatisfaction with the experience. I will not send my child to camp unwell. If my child is ill I will cancel the registration, and the cancellation terms above apply. I agree to the camp rules and health policies published on the camp website at the time of the session. Failure to follow them may result in a registration being canceled or a child being sent home.
